Sanjay Sharma, BSc, MD, FRCS, MSc (Epid), an Associate Professor of Ophthalmology and Epidemiology at Queen's University completed his medical education at Wills Eye Hospital and at Massachusetts Eye and Ear Infirmary. Dr. Sharma has authored over 150 articles that have appeared in the peer-reviewed literature, numerous book chapters and books- including the American Medical Association Press’s, Evidence Based to Value Based Medicine. Dr. Sharma is a co-editor-in-chief of Lippincott Williams & Wilkin’s Evidence-Based Ophthalmology/Eye Care, and has received numerous national and international grants and awards for his research in eye disease, including those from the Canadian Institutes for Health Research, the Canadian Foundation for Innovation and the American Academy of Ophthalmology. |

Brian Arthur MD, FRCS, is an Assistant Professor and the Director of the Pediatric Ophthalmology and Motility Service and the Program Director for the Department of Ophthalmology, Queen’s University, Kingston, Ontario. His areas of interest are in methods of assessment of visual acuity in infants and children, binocular visual sensory testing and localization of the neurophysiologic defect in ambloyopia. He has membership in a number of professional groups including the American Association for Pediatric Ophthalmology, the Pediatric Eye Diseases Investigator Group and the International Orthoptic Association. He has written several articles and has received an Honour Award from the American Association for Pediatric Ophthalmology and Strabismus. |

Jeffrey A. Bakal BSc, MSc, PhD, is a senior biostatistician with the Department of Cardiology at the University of Alberta and a research associate with the Cost-Effective Ocular Health Policy Unit at Queen's University. He completed his PhD jointly with the Department of Mathematics and Statistics and with the School of Physical Health and Education at Queen's University. He has worked on the methodology and analysis of several international studies in business strategy, ophthalmology, cardiology, in geriatric medicine and the analysis of kinematic data resulting in several peer reviewed articles and conference presentations. His current interests are in developing statistical methodology for time-to-event data and the development of classification tools to assist in the patient screening and patient decision making processes. |

Stephanie Baxter, MD, FRCSC, an Assistant Professor of Ophthalmology, serves as the Director of Undergraduate Ophthalmic Education at Queen’s University. Dr. Baxter, a specialist in Cornea and External Diseases completed her post-graduate fellowship training at Wills Eye Hospital in Philadelphia. Dr. Baxter is a member of several professional societies, including the Cornea Society, the American Academy of Ophthalmology, and the American Society of Cataract and Refractive Society. She has authored several articles that have appeared in the peer-reviewed literature and serves as a reviewer for the Canadian Journal of Ophthalmology. |

| George H.H. Beiko BSc.,BM. BCh. (OXON), FRCSC, is an Assistant Professor of the Department of Ophthalmology at McMaster University. Dr. Beiko has participated in a number of Phase 1 FDA trials of intraocular lenses. He serves as a reviewer or on the editorial board of several journals, including the Journal of Cataract and Refractive Surgery, Canadian Journal of Ophthalmology, Ophthalmology, Ophthalmic Practice, Cataract and Refractive Surgery Today, Indian Journal of Ophthalmology and Clinical and Surgical Ophthalmology. Dr. Beiko is a member of several professional societies, including the Canadian Ophthalmological Society, the Canadian Society of Cataract and Refractive Surgery, the American Academy of Ophthalmology, the European Society of Cataract and Refractive Surgery, and the American Society of Cataract and Refractive Society. He has been nominated to be a member of the International Intraocular Implant Club. Dr. Beiko has published extensively and presented both nationally and internationally, on cataract surgery. He is the current Canadian Hospitaller for the St. John’s Ophthalmic Hospital in Jerusalem. |

 | | Stephen Brodovsky, MD, FRCSC is an Assistant Professor in the Department of Ophthalmology at the University of Manitoba. Dr. Brodovsky specializes in anterior segment and refractive surgery. He completed his post-graduate medical education at Queen’s University and was both an EA Baker and McLaughlin scholarship recipient while doing his corneal fellowship at the University of Melbourne. He has published a number of articles in peer reviewed journals and has a special interest in international ophthalmology. |

 | | Rob Campbell MD, MSc, BSc(Hons), FRCSC is an Assistant Professor in the Department of Ophthalmology at Queen’s University, specializing in glaucoma and anterior segment surgery. Dr. Campbell is the author of numerous peer-reviewed publications and book chapters, and has won numerous awards for excellence in clinical and basic science research. Dr. Campbell has received research and training grants from the E.A. Baker Foundation, the Glaucoma Research Society of Canada and the Natural Sciences and Engineering Research Council of Canada (NSERC). Dr. Campbell, a medical school gold medallist and valedictorian at the University of Ottawa, completed his post-graduate medical education at the University of Toronto and Moorfields Eye Hospital. |

Alan Foster Cruess MD, FRCSC is Professor and Head of the Department of Ophthalmology at the Dalhousie Faculty of Medicine and the Queen Elizabeth II Health Sciences Centre. He has been a lead clinical investigator in the area of macular degeneration and diabetic retinopathy and was the principle investigator of the Medical Research Council of Canada- funded Canadian Ophthalmology Study Group. In addition to his academic, administrative and clinical care responsibilities, Dr. Cruess has actively participated in biannual Northern Ontario tours of the CNIB Mobile Eye Unit from 1995-2003, and from 1996-2003 was the ophthalmologist for the diabetic retinopathy screening program among the Cree people of western James Bay in Northern Ontario. Dr. Cruess is a member of the Scientific Advisory Board of AMD Alliance International and was recently appointed in 2006 to the Data Safety Monitoring Committee (DSMC) of the AREDS II Study, a multi-centre National Eye Institute-sponsored clinical trial to further elucidate the role of dietary supplements in AMD. Dr. Cruess, the author of numerous peer-reviewed scientific articles, chapter and books, is the current President of the Canadian Ophthalmology Society. |

Sherif El-Defrawy MD, PhD (Pharmacology), FRCSC, an Associate Professor and Head of the Department of Ophthalmology, Queen’s University, is the immediate past President of the Canadian Ophthalmology Society. Dr. El-Defrawy, a member of several professional societies, including the American Academy of Ophthalmology, and the American Society of Cataract and Refractive Society serves many important provincial and national committees, including Ontario’s Expert panel on Cataract Surgery Wait Time, the Canadian Institute for Health Information and the Health Protection Branch of the Federal Government of Canada. Dr. El-Defrawy, the recipient of many teaching awards, including the Professional Association of Interns and Residents of Ontario’s (PAIRO) Excellence in Clinical Teaching Award, has received several grants for research in eye disease. |

 | | Cindy Hutnik BSc, MD, PhD, FRCSC, completed doctorate work at the National Research Council of Canada in Ottawa prior to her medical training at the University of Ottawa. This was followed by ophthalmic training at the University of Western Ontario and the University of Wisconsin. Dr. Hutnik has a full-time clinical glaucoma practice at the Ivey Eye Institute in London, and is the Chair of Research in the Department of Ophthalmology, University of Western Ontario. She has given a number of invited presentations as a key opinion leader to Canadian ophthalmologists and is a member of the international Tear Film and Ocular Surface committee of the Association for Research in Vision and Ophthalmology (ARVO). The author of many scientific articles that appear in the peer-reviewed literature, Dr. Hutnik is an associate scientist of the Lawson Health Research Institute. She has received a number of teaching and research awards, most recently being recognized as an "alumni in the early stages of their career who have distinguished themselves through career successes and notable achievements” from the University of Windsor. |

| Peter J. Kertes, MD, CM, FRCSC is a vitreo-retinal surgeon and an Associate professor of ophthalmology at the University of Toronto. He has co-edited two books, Clinical Trials In Ophthalmology: A Summary and Practice Guide and Evidence Based Ophthalmology: Clinical Trials and Beyond, has written numerous chapters, and has been the author or co-author of dozens of peer-reviewed publications. He has received research support from multiple sources including an E.A. Baker Foundation Grant from the Canadian National Institute for the Blind. Dr. Kertes serves as the head of the retina section for the Canadian Ophthalmologic Society. He has also been recognized for his mentorship with several resident teaching awards. Dr. Kertes also has a special interest in international ophthalmology and has participated in volunteer humanitarian missions to Dominica, India, China, and Russia. |

Vladimir Kratky MD, FRCSC is an Associate Professor in the Department of Ophthalmology at Queen’s University, who specializes in ophthalmic plastic and reconstructive surgery. He is an examiner for the Royal College of Physicians and Surgeons of Canada. Dr. Kratky is a member of numerous professional memberships pertaining to oculo-plastics including, ASOPRS, the European Society of Oculoplastic and Reconstructive Surgery as well as the International Society for Orbital Disorders. Dr. Kratky, the author of numerous peer-reviewed publications, has received a US Patent for novel method to quantify globe proptosis. Dr. Kratky is a Past-president of the Canadian Society of Ophthalmic Plastic & Reconstructive Surgery. |

Paul Rafuse, MD, PhD, is a glaucoma specialist who is an Assistant Professor in the Department of Ophthalmology and Visual Sciences at Dalhousie University. He obtained his MD from The University of Calgary and his PhD in Pharmacology from the University of Alberta. Dr. Rafuse received his Residency training in Ophthalmology at the University of Western Ontario and did a Clinical Fellowship in Glaucoma at the Harvard University’s Massachusetts Eye and Ear Infirmary.Dr. Rafuse is a Past President of the Atlantic Provinces Ophthalmological Society and has served on the Board of the Canadian Ophthalmological Society as Chairman of the Council on Continuing Professional Development and presently serves as the Treasurer. He is the current President of the Canadian Glaucoma Society. Dr. Rafuse is a busy clinician and resident teacher who is actively involved in research in glaucoma. |

Gaurav Shah MD, an attending surgeon at the Barnes Retina Institute who specializes in diseases and surgery of the retina, vitreous and macula. Dr. Shah completed medical school at the University of Illinois College of Medicine in Chicago. He traveled to Santa Clara, California for internship at the Kaiser Permanente Medical Center, and was a resident at the University of Minnesota, Department of Ophthalmology for 3 years. His 2 years of fellowship were spent at the renowned Wills Eye Hospital under the direction of Dr. William Benson. Dr. Shah is currently an active investigator in numerous clinical trials dealing with macular degeneration, diabetic retinopathy, Uveitis and AIDS. The author of over 90 book chapters or articles that appear in the peer-reviewed literature, Dr. Shah has received significant awards from the American Society for Retina Surgeons, the Vitreous Society and the American Academy of Ophthalmology.Dr. Shah is the co-director of the Fellowship Program with the Barnes Retina Institute and currently serves as a reviewer for Archives of Ophthalmology, British Journal of Ophthalmology and Ophthalmic Surgery, Lasers and Imaging. Dr Shah is also an examiner for the American Board of Ophthalmology. |

| James H. Whelan BSc, MD, FRCSC is an Assistant Professor in the Deparment of Surgery at Memorial University of Newfoundland. Dr. Whelan specializes in Vitreo-Retinal surgery. He completed post-graduate medical education at the Wills Eye Hospital in Philadelphia. Dr. Whelan has published and presented articles on Genetic and Surgical Retinal disorders in peer reviewed journals and invited lectures. He is a former Detweiler traveling fellow with the Royal College of Surgeons of Canada. He is also a volunteer faculty with Orbis international. |
